How do I order a certified copy of a birth certificate?

 


The Louisiana Vital Records Registry maintains birth records only for births that occurred in Louisiana. Currently, the Louisiana Vital Records Registry maintains birth records from 1907 to the present.

Louisiana is a "closed record" state. This means that birth and death certificates are not public records.

How to order a birth certificate

In person: Complete an application for a certified copy of a birth certificate, bring your photo identification and correct fees to a Vital Records walk-in Service Center; a list of the service centers located throughout the State is available at:

By mail: Mail the completed application for a certified copy of a birth certificate, a copy of your photo identification and correct fees to:
